Physical evaluations will be administered for any Lakeview Middle School student-athlete in grade 6 and grade 7 on Wednesday May 29, 2013. These physical evaluations cost $20 and a form must be filled out by a parent/guardian in advance of the evaluation. Forms can be picked up in the main office at the Middle Schoolor it can be downloaded under the "Forms" section to the right.
